1. Referral needed. Before seeing the specialist, family physician needs to fax a referral to our clinic at 416-335-1719.
2. Referrals can be downloaded from our website www.TCDCclinic.ca and fax it to our clinic at 416-335-1719.
3. When we receive your referral form from your family physician, our reception will contact with you to confirm your appointment date and time.
4. Please bring your valid health card and all the current medications you are taking when you see the specialist at our clinic.
5. Please arrive 10 - 15 minutes earlier than the appointment time for registration.
